DO I NEED A DOCTOR’S REFERRAL?
At the FREE telephone or online consultation, we will
determine whether your infant, child or adolescent needs
pediatric physiotherapy services.
After the initial assessment at home, the computerized assessment will be sent to your doctor, requesting a
prescription for ongoing pediatric physiotherapy services.
WHAT IS THE PROCEDURE
FOR COVID-19?
Home Visits will NOT take place if anyone in the household
has COVID symptoms or have been in contact with anyone
with COVID 19.

24 hours prior to the home visit, you'll be sent a
questionnaire to fill out regarding symptoms.

The physiotherapist will wear a mask, gown, safety glasses and gloves during the visit. The protective gear will change with each client. Parents are required to wear a mask if the 2 metre distance is not possible.
WHAT IS YOUR NEW PATIENT PROCEDURE?
Once you contact me, you will be sent an encrypted intake
form, to outline your child's medical history, your concerns
and goals. You will sign a confidentiality agreement & parent's rights form.

We'll set up an online consultation to determine need for PT services.

We'll book your Initial visit within 1 week. You'll receive your assessment writeup and the treatment plan within 24 hours. Ongoing visits, if needed will then be booked

I'll communicate with your physician & other team members,
as needed.

WHAT TYPES OF HEALTH INSURANCE DO YOU ACCEPT?
Payment for home care physiotherapy services for your infant or child is at the time of the visit, but PACE Physio will send the assessment and treatment notes to the insurance company so you can be reimbursed for these physiotherapy services.
